Applying to Register with the Practice
To register you will need to fill out a Registration form and Medical Questionnaire for each member of the family.You will need to telephone the surgery to make a New Patient Appointment and when you attend you will be asked to sign the registration form to confirm the details are correct. You will not be registered with the Practice until you have attended this appointment and the GP has accepted your application.
When registering you will be asked to provide 2 proofs of identity, one of which should show your address, and one should show photographic identity.
Unless you have complex medical problems or take multiple medications, you will be asked to make a New Patient Appointment with the Practice Nurse rather than the Doctor. If you prefer you may make this initial appointment with the Doctor. Should you fail to keep this Registration Appointment without letting us know, you may not be accepted onto the Practice list.

Note that by sending the form you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.
You may fill out a registration form on-line, print it and bring it in with you on your first visit to the practice, alternatively you can print a blank form, fill it out manually and bring it in with you on your first visit to the practice. We will need a hard copy for your signature.
